Community Health Worker Initiative


Health360 is partnering with the Connecticut Department of Public Health (DPH) and three local hospitals, including Charlotte Hungerford Hospital, St. Mary’s Hospital and Danbury Hospital, to utilize Community Health Workers as an integral part of the WISEWOMAN Program (Well-Integrated Screening and Evaluation for Women Across the Nation).

WISEWOMAN is a program at the Center for Disease Control and consists of 22 CDC-funded programs in 21 states including Connecticut.

Heatlh360 is recruiting a Community Health Workers for full-time employment at Charlotte Hungerford Hospital, St. Mary’s Hospital and Danbury Hospital. This unique opportunity starts with a three month full-time paid internship ($15/hour) and transitions into full-time employment at the hospital starting in January 2016.

CT AHEC Network Statewide Initiative
Community Health Worker Initiative is a Connecticut AHEC Network statewide initiative lead by Southwestern CT AHEC.
Under the direction of Southwestern AHEC, the four centers of the CT AHEC Network are working with the Connecticut Department of Public Health to place well trained and supported Community Health Workers at 13 Hospitals and Health Centers across the state. Community Health Workers will be trained with AHEC’s Community Health Worker Core Competency Curriculum. AHEC will support the Community Health Workers by providing supervision, additional training and other support needed to ensure robust community participation in the WISEWOMAN program.
